Health History

Before your initial session, you will be asked to complete a Client Intake Form. To ensure proper therapy at every subsequent session it is the client's responsibility to always inform the massage therapist of new and existing health conditions.

Confirmations

Appointments should be confirmed the day before the scheduled massage.

Cancellations and No-shows

To avoid a cancellation fee of the full service price you are required to cancel an appointment at least 48 hours in advance by phone. This allows the opportunity for someone else to schedule an appointment. Failure to do so will result in charging for the session on your credit/debit card. Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a "no-show." No shows will be charged the entire amount of session scheduled to your credit/debit card.

Tardiness

Appointment times have been arranged specifically for you. If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. If you are late for your massage appointment, you will be provided as much massage time as available within the scheduled time slot. You will, however, still be responsible for the full amount for your scheduled session.

Payment

Payment is due upon receipt of service. Appointments may be required to be held on a credit card. Cash, local checks, Master Card, Visa, Discover are acceptable forms of payment. Please make checks payable to DreamScapes Massage.

Sexual Misconduct

Sexual misconduct is forbidden. Client understands that any illicit or sexually aggressive remarks, advances or gestures will result in the immediate termination of the session and will be liable for full payment of the scheduled appointment.

Physician Referral

A physician referral is generally not required in order to receive massage therapy. After speaking with your therapist and reviewing health history information, a determination will be made prior to your session if a doctor's note is required.
NOTE: Massage Therapy is not a substitute for medical examinations and/or diagnosis. It is recommended that you see a physician for any physical ailment you have.

Residential In-Home Visits

DreamScapes Massage also offers the comfort and convenience of residential in-home visits. All policies apply to in-home visits as if you were receiving a session in the office. An additional charge will apply.

Gift Certificates

Gift Certificates must be presented at time of appointment and will not be replaced if lost or stolen. Gift Certificates expire 6 months from date of purchase. After expiration, gift certificates can be renewed for a $25.00 fee.
